Overview

The HCP Care Manager will manage the implementation and delivery of support and maintenance for clients receiving the Bolton Clarke Home Care Packages funded by the Commonwealth Government, Department of Human Services. This includes comprehensive assessment, monitoring within a professional and case management framework, coordinating delivery and brokerage of services within the package budget, and communicating with family and other service providers as required. It offers a unique mix of computer-based care work and clinical care, combining administrative and clinical skills.

Responsibilities

* Provide comprehensive client assessment and develop care plans to support the client at home.
* Apply advanced holistic care skills within a case management framework.
* Demonstrate understanding of the target population and their needs, from care coordination to complex case management.
* Demonstrate expertise in budget management within a CDC framework.
* Work within the required legal framework underpinning the Home Care Packages Programme.
* Liaise with multiple service providers to ensure care recipient needs are met in line with objectives and timelines.
* Support the care recipient with a reablement and wellness focus.
* Participate in quality improvement processes, including case reviews and team meetings.
* Maintain case files, documentation and statistics according to procedures.
* Participate in support and supervision activities supportive of the Home Care Packages within the Packaged Care Program.

Qualifications

* APHRA registration essential.
* Bachelor of Nursing/RN with experience highly desirable or recent RN graduate with previous nursing care and/or care plans, budgets and case management experience.
* Relevant case management and aged care experience in a similar role.
* Highly developed assessment skills and the ability to creatively problem solve complex care situations.
* Understanding and experience in the delivery of health services and/or community-based services, as well as complex case management.
* Proven ability to contribute to and work as part of a dynamic team and also operate independently.
* Demonstrated skills in networking, liaison and the development and maintenance of strong links with relevant service providers and users.
* Valid working rights in Australia (Passport, Birth Certificate, etc.).
* Police Check (or willingness to undertake the check).
* Willing to obtain annual flu vaccinations.
* Evidence of two COVID-19 vaccinations.
* Own vehicle/licence.
