About the opportunity

We are looking for a caring and community‑minded Support Coordinator to join our Aftercare Service providing non‑clinical practical support to people recovering from a suicidal crisis or attempt with a strong focus on older people.

In this role you’ll walk alongside participants for up to three months, helping them regain stability, build confidence and connect with the right health community and social supports. You’ll also work closely with aged care providers, lifestyle villages and mental health services to strengthen pathways of support and raise awareness of suicide risk factors in older age.

What you’ll do :

- Provide practical compassionate support tailored to each individual.
- Develop and review recovery‑focused support and safety plans.
- Build strong referral pathways for older people into Aftercare and community services.
- Partner with aged care homes and lifestyle villages to raise awareness and support staff.
- Ensure all participants feel respected, safe and supported.

What we’re looking for :

- Solid people skills and the ability to listen with empathy.
- Experience in community services aged care health or a related field.
- Confidence working with diverse groups and building partnerships.
- Organised, reliable and able to keep accurate records.

To be successful in this role you will need :

- Relevant qualifications or commensurate experience in working in the mental health or community services sector, generally a Diploma level qualification.
- Demonstrated experience and/or transferable skills/knowledge in working with older people living with mental health conditions.
- Experience in working with people who have experienced suicidal crisis/behaviours or experience in dealing with severe personal trauma.
- Current valid Driver’s License and the ability to undertake some travel and be away from home overnight.
- Appropriate IT skills including knowledge of and experience with Microsoft Office Suite.
- NDIS Worker Orientation Module Certificate (free online course).
- Satisfactory National Police Records Check.
- NDIS Worker Screening Check.
- Working with Children’s Check (Employee / Paid).
