Care Coordinator / Clinical Care Coordinator

What You'll Do
* Assess residents and develop individual care plans
* Coordinate daily clinical care across nursing and care staff
* Monitor resident health and wellbeing
* Oversee medication management and clinical procedures
* Support and guide care teams in service delivery
* Liaise with doctors, families, and allied health professionals
* Ensure compliance with Aged Care Quality Standards
* Manage documentation, incidents, and care reviews
* Support admissions, transitions, and care planning updates
* Provide leadership during clinical emergencies
What We're Looking For
* Registered Nurse (RN) with AHPRA registration (typically required)
* Experience in aged care, hospital, or community nursing
* Strong clinical assessment and care planning skills
* Leadership and team coordination experience
* Knowledge of dementia and palliative care
* Robust communication and documentation skills
* Understanding of Australian aged care regulations
Pay: From $8,000.00 per month
