Pride Disability Services Pty Ltd is a NDIS Registered provider. They provide individualized care and support to their participants with disability mainly in Melbourne.
Role
This is a full‐time position with an annual salary of $77,000 to $80,000, depending on experience, plus superannuation. The role will be based in Melbourne, VIC.
Responsibilities
* Assess the unique needs of clients to tailor individualized care plans.
* Plan, develop, and implement educational, training, and support programs to address clients' needs.
* Conduct client interviews to comprehensively understand and assess the nature and extent of difficulties.
* Monitor and track the progress of clients, ensuring regular assessments and documentation.
* Facilitate referrals to external agencies and resources that can provide additional assistance and support.
* Evaluate community needs and available resources related to health, welfare, housing, employment, training, and other services for individuals with disabilities.
* Liaise with community groups, welfare agencies, government bodies, and private businesses to address community issues and promote awareness of available resources.
* Provide support to families and deliver education and care services for children and disabled individuals in various settings, including adult service units, group housing, and government institutions.
* Assist young people in resolving social, emotional, and financial problems related to their disabilities.
* Prepare submissions for funding and resources, outlining the specific needs and goals of the disability care services organization.
* Generate reports for government bodies and other agencies to provide updates on the organization's activities and outcomes.
* Advocate for the rights and needs of individuals with disabilities within the community and relevant institutions.
* Identify and refer residents to external agencies for additional support when necessary.
Requirements
* Must hold a minimum Cert IV in Disability and Diploma in Community Services.
* Minimum 2 years' experience supporting participants with complex disability needs, including psychosocial disability, intellectual disability, autism, acquired brain injury and physical impairment.
* At least 2 years' experience implementing positive behaviour support strategies, behaviour observation records, escalation plans and restrictive practice reporting requirements.
* Minimum 2 years' experience coordinating high intensity support needs, including manual handling, dysphagia support, medication prompting, seizure response, continence support and mealtime management.
* At least 2 years' experience preparing detailed case notes, shift reports, incident reports, progress summaries, risk reviews and participant outcome evidence for NDIS audits.
* Willing to learn new skills and adapt your approach to meet the individual needs of the person you support.
* Effective prioritising, problem solving, time management and decision-making skills.
* Demonstrated ability to assist with daily programs and organising activities/excursions.
* Effective communication skills with clients, families, and healthcare professionals.
* Cultural sensitivity and respect for diverse backgrounds.
* Have positive past employment work references.
