Based in Manning Valley on the beautiful Lower Mid North Coast, you'll join the Hunter New England Health Aged Care Services team – a values‐driven service dedicated to excellence, cultural respect and person‐centred care. This role offers the best of both worlds: a fulfilling professional challenge and the relaxed regional lifestyle that Taree is known for. You'll work across hospital, community and residential settings, collaborating with medical, nursing, allied health and community partners to deliver seamless support across the aged care journey.

About the Role

As a Nurse Practitioner (Aged Care), you'll bring advanced clinical knowledge and leadership to provide high‐level, evidence‐based care to older people across a variety of settings. Working autonomously within your endorsed scope, you'll make clinical, diagnostic and prescribing decisions, while collaborating closely with geriatricians and the multidisciplinary team to design and implement integrated management plans that improve safety, quality and the experience of care.

You'll also play a key role in supporting the Residential Aged Care in Reach (RACI) program, helping to reduce hospital admissions through early intervention, pre‐emptive assessment and coordinated home and clinic‐based care. The role focuses on managing chronic and complex conditions such as cognitive impairment, dementia, sleep disorders, depression, anxiety, functional decline and falls.

Beyond direct care, you'll contribute to service planning, quality improvement and education, supporting the ongoing growth of aged care nursing practice across the region.
