About the Role

As the Manager of Wellness & Reablement Services, you will lead a dedicated team delivering innovative, person-centred services under the Commonwealth Home Support Program.
In this role, you will drive the delivery and continuous improvement of Wellness and Reablement services that support older people to maintain independence, wellbeing, and quality of life. You will champion restorative and reablement-focused approaches, ensuring services are responsive, effective, and aligned with best practice standards.
Working closely with internal teams and external stakeholders, you will build strong collaborative partnerships, support service growth, and foster a high-performing team culture grounded in accountability, empowerment, and care. This is a full-time role based at either our Launceston or Warrnambool office, Monday to Friday.
In this leadership position, you will bring a minimum of 4 years' experience in aged care or home care services, with proven ability to lead service delivery in complex, regulated environments.
You will be confident in leading teams, including supervision, performance development, and change management. Strong communication and influencing skills will enable you to build effective relationships, foster collaboration, and drive a solutions-focused approach to service delivery. A sound understanding of the Home Care Packages Program, Aged Care Reforms, Aged Care Quality Framework, and Home Care Standards is essential.
No two days are the same! As Manager - Wellness & Reablement Services you will:
* Lead operational delivery across Home Care programs, ensuring high-quality, consistent service outcomes
* Ensure full compliance with Aged Care Quality Standards, Code of Conduct, accreditation, and funding obligations
* Design and deliver integrated, consumer-focused care models in partnership with senior leadership
* Identify and drive innovation, service development, and growth opportunities aligned to community need
* Own program performance, including planning, monitoring, evaluation, and reporting against key targets
* Lead financial performance, ensuring budgets are managed effectively and services remain sustainable
* Build and lead high-performing teams through effective recruitment, development, and performance management
* Embed a culture of continuous improvement, accountability, and exceptional consumer experience
* Oversee risk, compliance, and quality systems including incidents, complaints, and regulatory requirements
* Contribute to strategic direction, business development, and growth initiatives across the Home Care portfolio
About You
You'll thrive with us if you have:
* Proven experience leading and delivering complex Home and/or Community Care programs
* Strong leadership capability with a track record of building and developing high-performing teams
* Confidence interpreting and applying policy, legislation, and regulatory requirements in aged care settings
* At least 4 years' experience in aged care, home care, or a related field
* Excellent communication skills with the ability to build strong relationships and influence outcomes
* A collaborative leadership style that fosters positive team culture and engagement
* Demonstrated ability to drive service growth and identify new opportunities within a portfolio
* Strong understanding of the aged care sector, including Aged Care Quality Standards, Code of Conduct, and accreditation requirements
* Well-developed problem-solving and conflict resolution skills, with a solid grasp of risk, incident, and hazard management
* Strong digital and systems capability, including confident use of reporting tools, databases, and information systems
