Join our dynamic team at Central Coast Local Health District (CCLHD) as an Inpatient Social Worker (Oncology & Palliative Care) Gosford Hospital and be a crucial part of delivering optimal health outcomes and psychosocial care.

With an understanding of the psychosocial challenges that accompany chronic and acute illness, you'll provide comprehensive assessments, crisis management, and brief interventions tailored to meet the unique needs of each individual and their support network. Your ability to adapt communication styles, work collaboratively in multidisciplinary teams, and prioritize competing demands will ensure the delivery of high-quality care in a fast-paced environment.

This full time inpatient Social Work position specialises within the Oncology and Palliative Care Social Work Team at Gosford Hospital, will enable you to further your current knowledge and gain a broad breadth of Inpatient Social Work field of practice experiences. From providing emotional support including grief and loss, working with families and carers to facilitating smooth transitions through the healthcare system, you'll play a pivotal role in ensuring patients and their families receive the support they need during difficult times.

**About the Role**

The position is part of the Inpatient Social Work team based at Gosford Hospital that provides a professional social work service to patients and their family/carers to ensure the provision of optimal health outcomes for patients consistent with CCLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. This position will work alongside multidisciplinary teams to deliver high quality care in a dynamic team environment.

The Social Worker is to provide assessment, crisis and brief intervention, counselling and case planning to assist patients/families/carers to deal with the emotional impact of illness, to facilitate smooth transitions through the hospital system and to access appropriate community services. The Social Worker will work with the multidisciplinary team, senior managers and other inpatient social workers to ensure the delivery of effective and proactive social work services and psychosocial care.

**About You**

You have:

- Tertiary qualification in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to Inpatient Social Work.
- A well-developed understanding of the psychosocial issues that arise for patients and their families during chronic and acute illness and trauma. Preferably demonstrated social work experience in the clinical specialty area where applicable.
- Demonstrated experience in providing psychosocial assessment and crisis management, within person centred, trauma informed and brief intervention models and preferably within a health clinical setting and or within the clinical specialty where applicable.
