BCR Communities is a not-for-profit community organisation proudly serving communities within the Illawarra and Shoalhaven since 1991. We have been providing Youth & Community and Disability Services within the Bay & Basin region, the home of our Activities Hub, Community Centre and Bay & Basin Community Radio Station 92.7FM, for over 30 years. BCR is also a local leading provider of Aged Care services, including Commonwealth Home Support (CHSP), Home Care (HCP) and Short-Term Restorative Care (STRC).

We are seeking an experienced and suitably qualified Clinical Leader for the role of Home Care Clinical Lead to provide clinical oversight and operational clinical guidance to ensure high quality, safe and meaningful client centred care across our aged care services.

Reporting to the Executive Manager of Aged Care Services (ACS), the Clinical Lead is responsible for overseeing the day-to-day clinical care and supports in our home care services, with a view to manage, monitor and review the program, ensuring compliance with Aged Care Quality Standards and the BCR Care and Clinical Governance Framework. The role will perform higher duties in the absence of the Executive Manager of ACS.

The successful applicant will develop a rapport and work closely with the Care Partners, Clinical staff and the BCR Leadership team to champion high quality and safe practice supporting continuous improvement. They will also coach and mentor our team members to deliver quality outcomes for clients and build a strong and positive culture.

Key responsibilities will include:

Contributing to the enhancement, revision, and execution of clinical policies and procedures

Supporting the Executive Manager ACS to sustain performance improvement as well as quality improvement activities, including participating in clinical audits across the service

Providing supervision and support to the clinical team for general practice, assessments, care plan development, monitoring and review for clients

Enhancing, devising and implementing competency-based training and coinciding procedures relating to clinical care service wide

Completing in-home clinical assessments, care planning and clinical support as required

Reporting and attending committees and meetings to contribute clinical expertise and advice

Managing clinical incidents, complaints and investigations, reporting to the Executive Manager ACS and Aged Care Quality and Safety Commission as required (SIRS)


About You:

Registered Nurse with current AHPRA registration

Previous clinical leadership experience, preferably within a community health or aged care setting

Demonstrated clinical knowledge and skills to provide safe and effective clinical care

Excellent skills in assessing, documenting and implementing client care plan goals

Experience in community care/home based care services is desirable

Excellent communication and interpersonal skills.

Highly developed organisational and prioritisation skills.

Demonstrated self-awareness and ability to work autonomously yet cooperatively in a team

Proactive and focused on continuous improvement and experience in ensuring compliance to aged care standards and regulations

Unrestricted Drivers Licence, WWCC, NDIS Worker Screening, and satisfactory National Police Check

Vaccinated against COVID-19 (minimum of a primary dose)
