Capecare is a leading provider of aged care and accommodation services for seniors across the Southwest region of Western Australia, operating two residential facilities in Busselton and Dunsborough on the scenic Geographe Bay.
Role Overview
As a Primary Care Coordinator (Clinical Relationship Manager) based at the Dunsborough facility, you will foster partnerships between Capecare residential aged care homes, general practitioners, allied health providers, pharmacists and other community health professionals to ensure seamless, patient‐centred care for residents.
Key Responsibilities
* Develop strong collaborative relationships with GP's, allied health professionals and pharmacists to create a partnership‐in‐care approach.
* Collaborate with GP's to facilitate efficient, timely and responsive primary care for residents.
* Facilitate communication between GP's, Capecare residential staff, residents and family/carers, keeping all parties informed and involved in the care process.
* Support knowledge‐building and skill development among clinical and care staff.
* In consultation with the Quality & Clinical Governance Manager, develop clinical protocols with GP's to guide decision making, triage, assessment and clinical escalation.
* Maintain oversight of GP's providing treatment and care to residents.
* Develop and manage an ongoing Implementation Working Group, in line with the GPAIC guidelines, comprising Capecare senior clinical leaders, GP's and the WAPHA Activity Lead.
Qualifications
* Bachelor of Nursing – AHPRA registration.
* Postgraduate qualifications in dementia care, palliative care, primary care or mental health nursing (desirable).
* Minimum of 5 years' experience in aged care.
* Experience in dementia and palliative care.
* Demonstrated knowledge of the Age Care Act and Standards.
