Job Description:
We are looking for a compassionate and reliable Support Worker to join our team. In this role, you will provide person-centered support to individuals with disabilities, helping them live independently and improve their quality of life. You will assist participants with daily activities, promote their wellbeing, and support them in achieving their personal goals.
Key Responsibilities:
* Assist participants with daily living activities such as personal care, meal preparation, and household tasks
* Support participants in community access, appointments, and social activities
* Encourage independence and help participants develop life skills
* Follow individual support plans and provide person-centered care
* Maintain accurate documentation and progress notes
* Ensure a safe and supportive environment for participants
* Communicate effectively with participants, families, and team members
Requirements:
* Experience in disability support, aged care, or community services (preferred)
* Certificate III or IV in Individual Support, Disability, or a related field (preferred)
* Current First Aid and CPR certification
* Valid driver's license and reliable vehicle (preferred)
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
Pay: From $35.00 per hour
Work Location: In person
