Rise is hiring part-time Support Workers to walk alongside our Mental Health clients within the Community.

The need-to-know…
At Rise, Support Workers provide practical, person-centred support to people living with psychosocial disability, helping them build skills, confidence and independence in everyday life. The role is centred on building genuine, consistent relationships and supporting people to work towards goals that are meaningful to them, at their own pace.

This is a community-based position, with travel required across the Perth metro area. Once you have joined the team and established ongoing client connections, part-time hours will be guaranteed through an agreed Hours of Work arrangement. Shifts will be available Monday to Friday, between 8:00am and 5:30pm.

No two days look exactly the same, but your work will include:

* Supporting people to work towards goals outlined in their individualised plans
* Using person-centred, strengths-based and recovery-oriented approaches
* Assisting with daily living tasks as outlined in support plans
* Supporting people to attend appointments, meetings and community activities
* Helping people explore interests, build routines and strengthen social connections
* Encouraging independence through shared decision-making and problem solving
* Linking people with community services, clinical supports and local resources
* Providing clear, timely progress notes and reports using client management systems
* Recording support delivery, incidents and relevant observations
* Supporting new team members through buddy shifts and shared learning

The need-to-have…
Here At Rise, We Nurture Potential, But We Do Need You To Have a Few Tricks Up Your Sleeve Including The Following

* A related qualification (Disability, Aged Care, Allied Health or Community Services) and/or relevant hands-on experience
* Experience working within a mental health recovery approach
* A practical, strengths-focused way of supporting people to live well and build independence
* Experience helping shape, deliver and review plans that support real, meaningful goals
* The confidence to work independently, balanced with a genuine team mindset
* Sound judgement, good initiative and the ability to keep clear, professional boundaries
* Driving licence with use of a reliable vehicle for work purposes

Formalities (The serious stuff)
Appointments to this position require a NDIS worker screening check and Working with Children Check. Costs for new checks will be covered and/or reimbursed by Rise.
