About the role
The Service Coordinator role develops strong collaborative relationships with customers and their families, acting as the primary point of contact for core support services.

As our new Service Coordinator, you are responsible for monitoring and optimising the health, well-being, citizenship, safety & satisfaction of customers, engaging other service providers internally or externally where necessary.

How will you make a difference?

The Service Coordinator supports customers to choose how their funding should be used, ensuring services purchased reflect customer choice & funding available.

As the Service Coordinator, you will:


• Act as primary liaison with customers/families

• Deliver & role model excellent customer service

• Provide quality practice leadership & coaching

• Facilitate and maintain high-quality Customer Support Plans and associated documentation

• Responsible for customers' NDIS pre-planning, goal progress, progress reports, plan utilisation and optimisation.

• Manage operating budgets and meet relevant Key Performance Indicators.

• Manage rosters and customers' funding packages efficiently and effectively.

• Ensure documentation & customer information are kept up to date in a timely way, including CMS data integrity.

• Report, manage, evaluate and monitor incidents, investigations and complaints in a timely manner whilst focusing on prevention & continuous improvement. Participate in Quality & Safeguarding Commission activities.

About You

• A tertiary qualification (or equivalent) in Health, Disability, Social Services, Behavioural or Sciences is desirable.

• Minimum 3 years' experience working with people with a disability in a similar role and/or working for a community organisation in a similar role.

• A good understanding of contemporary practices within the disability sector, including active participation, supported decision making, self-determination and citizenship.

• Highly developed verbal and written communication and interpersonal skills; able to consult with a wide range of people.

• Strong cultural/diversity awareness and sensitivity.

• Well-developed planning and organisational skills.

• Understanding of and commitment to principles of NDIS and Quality & Safeguarding Framework.

• Committed to working collectively towards AbilityWA's purpose and strategic initiatives.

• Has the right to work in Australia with the ability to obtain NDIS Workers Screening Clearance, National Police Check & Working with Children's Check.

• Current nationally accredited First Aid certificate.

• Current and valid 'C' class driver's license.

• COVID-19 Vaccination Certificate.

• Promotes a safe work culture and complies with Work Health & Safety policies.
