About The Opportunity
Join a groundbreaking first-of-its-kind Equity Model of Care – Supporting Families through a Midwifery Group Practice and Child and Family Health model.
We're seeking passionate clinicians to join an innovative, equity-driven model of care designed to reach and support vulnerable families across the first **** days of life.
This whole-of-family approach centres on continuity of care, connection, and proactive care in trusted, community-based settings.
Responsibilities
Build trusted relationships through early, supported engagement and ongoing continuity of care.
Partner with families alongside a known midwife and child and family health nurse, delivering coordinated, evidence-based care from pregnancy to early childhood.
Empower families with seamless service navigation, ensuring the right support is delivered at the right time.
Use digital tools like Watch Me Grow – E to enhance communication, promote developmental milestones, and keep families engaged and informed.
Early career midwifery and nursing staff encouraged as part of this innovative model.
What You'll Be Doing
Provide Social Work services to consumers and their carers to ensure optimal health outcomes consistent with SWSLHD policies, procedures, and the NSW Health Code of Conduct.
The position also participates in professional development, clinical supervision, quality activities, and other relevant administrative duties.
Where You'll Be Working
SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
Services include prevention, early intervention, assessment, acute & post-acute treatment, health maintenance, and continuing care.
The team supports diverse populations, offering specialised programmes for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.
How To Apply
To be considered, please ensure you address the following criteria:
Social Work degree providing eligibility for membership of the Australian Association of Social Workers (AASW) and extensive post-graduate experience as a Social Worker in relevant clinical areas.
Demonstrated experience in prioritising a challenging and demanding workload, managing stressful situations, and applying advanced reasoning skills and independent judgement.
Advanced clinical knowledge and experience in comprehensive needs assessments with a focus on risk and safety planning for children, young people, and families.
Experience working independently and collaboratively within a multidisciplinary team.
Must possess an Australian C-Class driver's licence (including P1 or P2).
Licences from other states must be upgraded within three months of appointment.
