Support Coordinator/Psychosocial Recovery Coach

Location: Perth

Employment Type: Part-time

* Remuneration: $42.82 per hour
* Location: Perth Office & South Metro – outreach required
* Employment Type: part time (0.6FTE) with opportunity to extend to full time

The Position

Join a supportive team helping to establish a positive relationship with NDIS participants and their support network, while ensuring the implementation of their plans. This positions supports participants to identify and connect with supports in their local communities, build skills, overcome barriers, and to achieve their goals on their NDIS plans.

This role aims to promote participant's independence and confidence, to enable them to live their best lives.

Duties of the role will include (not limited to):

* Play an active role in supporting NDIS participants to build capacity to coordinate their NDIS plans, negotiate and connect clients to appropriate support and services;
* Identify, coordinate, manage and or facilitate a range of supports and support providers to meet identified needs;
* Regularly monitor expenditure and support participants to remain informed about their NDIS plan funding;
* Provide expert advice and consultation to NDIS participants and their families on the changing NDIS environment to build capacity to understand and navigate service systems.

About You

You have at least 2 years experience working in the mental health field providing participants with NDIS supports.

Minimum Qualifications

* Proof of Australian Working Rights
* Current Drivers License & Car Insurance
* A minimum Cert IV in Mental Health or Community Services with demonstrated experience providing Support Coordination.

Additional Requirements

* NDIS Worker Screen Clearance
* NDIS Orientation Module Certificate
* Working with Children Card
* First Aid Certificate
