Case Manager Job Description
The Case Manager provides crisis intervention, support services, and advocacy for adolescents who are experiencing homelessness or exploitation.
Main Responsibilities:
* Complete individual assessments of youth's needs and strengths
* Develop and review plans of care with youth
* Model and teach life skills
* Meet with clients regularly
* Act as liaison between youth and external providers
Requirements:
* Bachelor's degree in human services or related field
* 2 years of experience in human services or equivalent education
* Reliable transportation
* Experience with billable case management preferred
