We're looking for a skilled professional to join our team as a Stepped Care Coordinator. This exciting new initiative provides integrated, person-centred care across Central Queensland, Wide Bay, and the Sunshine Coast. Our ideal candidate will have extensive experience in mental health and/or AOD settings, including knowledge of systems, legislation, and brief-intervention approaches.

Job Description:

* To provide direct engagement and brief interventions to ensure smooth transitions and aftercare.
* To work closely with internal teams and external partners to deliver comprehensive care.
* To support clients by being their first point of contact and helping them navigate their care journey, ensuring a welcoming experience at every step.
* To oversee collaborative care planning, transitions between services, and aftercare.
* To build relationships with local hub partners and the lived experience workforce to foster community-driven care.


Key Responsibilities:

1. Lead the way for clients by providing direct engagement and brief interventions.
2. Collaborate with internal teams and external partners to deliver comprehensive care.
3. Support clients through their care journey.
4. Manage care processes to ensure seamless transitions.
5. Strengthen partnerships with local communities.


Requirements:

* Qualifications in psychology, social work, mental health nursing, counselling or a related field.
* A maintained relevant annual registration and/or membership of professional body (preferred).
