About the Role

Deliver clinical care in the home environment supporting clients to stay living in their home and connected to their community
Embody a person-centred approach, recognising and promoting the individual's unique goals and preferences
Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered
Orchestrate case conferences and maintain open communications with clients, families, carers and authorised representatives
Effectively prioritise client needs in collaboration with other medical and health professionals as required

About You
You demonstrate compassion, empathy and advocacy
Excellent communication skills, both verbal and written
Ability to work independently and demonstrate sound decision-making
Ability to work efficiently by optimising the use of technology and prioritising your time effectively across your caseload
Current AHPRA registration and Australian working rights
Valid driver's licence and a licensed, comprehensively insured, road-worthy motor vehicle
Compliance
It is a mandatory requirement for a team member working in the aged care sector to have a valid Police Check or a valid National Disability Insurance Scheme (NDIS) check, or potentially both, to undertake the inherent requirements of the role. Clearances must be verified prior to commencing employment.
