Employment Type : Permanent Full Time Position Classification : Registered Nurse Remuneration : $ 72,152.53 - $101,299.48 per annum plus superannuation Hours Per Week : 38 Requisition ID : REQ569823 Applications Close: Sunday 18 May 2025 Prince of Wales Hospital - Registered Nurse, Community Transition Aged Care Program Join an energetic Allied Health and Nursing team who are passionate about improving rehabilitation outcomes for patients upon discharge from Hospital. The Transitional Aged Care program (TACP) works towards improving the independence of individuals who are receiving TACP which is a home-based aged care rehabilitation program for up to 12 weeks. Where you'll be working Based in the Catherine Hayes Building on the Prince of Wales Hospital Campus Randwick. You will be visiting Clients in their own home and provide Registered Nursing interventions in a home based setting. You will assess and review the clinical health and well-being of the client participating in TACP. You will be liaising with the client’s general practitioner, reviewing and educating the client, on their medication management and routine. The Role You will conduct Nursing assessments in the client’s home and liaise with other treating members of the allied health team who are involved in the client’s care. Hospital avoidance is a priority of this program. You will visit clients in their own home and drive Hospital Fleet vehicles Transitional Aged Care Program is a home-based rehabilitation program for older people discharged from Hospital. The TACP Registered Nurse provides comprehensive assessment, care planning and case management including: referral, support and information to clients and their family/carers regarding long-term services for ongoing safe and appropriate care. Some of the key functions of the Registered Nurse/ Midwife is to: Assess, plan, organise, evaluate and deliver holistic quality nursing care in accordance with the Australian Nursing and Midwifery Council (ANMC) National Competency Standards competencies, NSW Health and South Eastern Sydney LHD policies and procedures, to achieve patient/client health outcomes. Selection Criteria: Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others. Demonstrated ability to proactively engage with patients/clients to enhance service delivery and to guide others in providing service excellence. Demonstrated ability to work autonomously and as part of a multi-disciplinary team and apply professional and ethical boundaries when dealing with complex situations. Demonstrated computer skills with reference to data collection, email and Microsoft Word, and proven ability to use health information systems. Detailed knowledge and understanding of risk management, safety and quality principles and the role of the Registered Nurse in applying these principles. Demonstrated recent experience as a Registered Nurse in the area of aged care and/or rehabilitation, and current professional practice knowledge related to working in the community as a Registered Nurse including demonstrated ability to perform comprehensive assessments and case management of older people, in particular those with dementia or with complex health care needs. Holds a current unrestricted NSW Drivers’ Licence.
