About this Role:
We are seeking a compassionate and dedicated individual who can deliver high-quality personal care and assistance with daily living activities, collaborate effectively with our Home Care team, and facilitate social outings and transportation services.
 * Providing personalized care and support with daily living activities
 * Collaborating with the Home Care team
 * Facilitating social outings and transportation services
 * Delivering in-home respite care guided by a tailored Care Plan
 * Monitoring and reporting on changes to clients' health through established protocols
 * Building strong relationships with clients and their families
This role requires a Certificate III in Individual Support or similar qualification, prior experience in Aged Care/Disability sector, excellent communication skills, empathy, and a valid driver's license with comprehensive car insurance.
