What's involved – The Palliative Care Clinical Nurse (PC CN) is an experienced professional with clinical knowledge in palliative care, symptom management and end-of-life care planning.
The position offers an exciting and innovative opportunity to collaborate with the Specialist Palliative Care Team.
This role requires the nurse to practice autonomously while providing leadership, support and education to patients, families and staff within Central West HHS.
The service operates five days a week and works alongside community-based services, Specialist Palliative Care Rural Telehealth (SPaRTa), GP's, hospital inpatient teams and multidisciplinary Allied Health services.
As a member of the Primary Health Team, the role will work collaboratively and more broadly with the team and across Central West HHS to ensure continued focus on strategic priorities and delivery of operational imperatives.
