South Western Sydney Local Health District's Transitional Aged Care Program (TACP) is seeking an Enrolled Nurse to join the residential based team. This is an excellent opportunity to contribute to a unique, multidisciplinary program that focuses on the reablement of older adults as they transition from hospital care into the community. TACP aims to deliver improved health outcomes and maximise quality of life for older people through coordinated, person centred care. In this role, you will work closely with the multidisciplinary team and relevant stakeholders to ensure holistic, high quality care for TACP clients and their families within residential aged care settings.
Are You the Right Fit?
You are an Enrolled Nurse with knowledge and experience working with aged care and/or community populations and a strong interest in supporting older adults through reablement focused care. You bring sound clinical reasoning skills across a diverse range of clinical areas and are confident in assessing TACP clients within residential aged care facilities, contributing to holistic goal setting, developing client centred care and intervention plans, and supporting effective discharge planning. You work well within a team environment and value collaboration, working closely with a Registered Nurse and other members of the multidisciplinary team to deliver safe, evidence based and person centred care.
Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.
1. Current Enrolled Nurse (Division 2) registration with the Nursing and Midwifery Board of Australia with no notations related to administration of medication.
2. Demonstrated Nursing knowledge and skills relevant to Community Nursing (e.g. skin assessments, risk identification, escalating clinical deterioration and so on).
3. Knowledge of and ability to work within a person-centered approach to nursing with a willingness to embrace change in the delivery of care.
4. Demonstrated ability to work independently with minimal supervision while effectively collaborating within a multidisciplinary team to achieve shared goals and deliver high-quality outcomes.
5. Applicant must possess an Australian C-Class driver's license (including P1 or P2)? Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
