We're Hiring: Full time Home Care Package Care Partner – Bundaberg
Monday to Friday | 8:30am – 4:30pm | Permanent Position

We’re currently recruiting for a community-based Care Partner (Clinical or Non-Clinical) to join our Sunshine Coast team, with the position based in Bundaberg.

Key Responsibilities
Conduct in-home assessments of participants
Provide ongoing case management, supporting both clinical and holistic needs
Monitor participant wellbeing and update care/case/health plans accordingly
Document and track participant goals, progress, and outcomes using person-centred, wellness, and reablement strategies
Manage participant budgets (Government Funding) and ensure compliance with documentation and reporting standards
Liaise with ACAT, My Aged Care, PRODA, and other regulatory bodies
Provide support, training, and guidance to support staff and colleagues
Assist with service scheduling and respond to urgent care placements or changes
(Clinical Care Partner Only): Occasional wound care duties
Advanced computer and administration skills (CRM proficiency required)
Excellent multitasking, prioritisation, and problem-solving abilities
Proven ability to work independently and as part of a team
Previous experience working to KPIs and tight deadlines
Current CPR/First Aid Certificate

NDIS Screening Card
Working With Children Check
Valid Australian Driver’s Licence
Desirable Qualifications
Previous experience in home care packages, community care, or aged care/health sector
Understanding of Home Care Package Program guidelines and compliance
Clinical Care Partner:
Registered Nurse (AHPRA) or
Enrolled Nurse (AHPRA)
