About The Role

In this role, you will be responsible for the provision of Care Management services to a case load of Home Care Package clients. You will be involved in the development and review of care plans and have oversight of the allocated funds of clients who are recipients of Home Care Packages and their carers within our community.

Work type: Fixed Term - Full Time (12 months)

Classification: Band 6 $95,760 pa plus super


Key Responsibilities

* Develop, implement, and review holistic care plans within required timeframes, including needs assessment, service agreements, risk assessments, and advanced care planning.
* Monitor participant needs and risks, review care notes, and evaluate service quality and outcomes through case conferencing and ongoing assessment.
* Ensure compliance with Support at Home program requirements, aged care standards (including the Aged Care Quality Standards), and legislative obligations, including mandatory reporting and incident management.
* Support routine problem-solving and continuous improvement activities by identifying and addressing gaps and inefficiencies.
* Coordinate service delivery by communicating with aged care workers, managing transitions between care settings, and ensuring participant needs and wellbeing are met.
* Collect and use participant data to inform service planning, support participants transitioning between aged care programs, and ensure responsible use of resources in line with funding guidelines.


About You

To be successful in this role, you will be an experienced aged care case manager who enjoys engaging with a diverse range of people, and with the ability to build rapport with clients and stakeholders. You will have a professional approach to your work, have high levels of resilience and the ability to be innovative and think outside the box when finding solutions to problems.


Essential Experience

* Tertiary qualification in a relevant field (e.g., aged care, nursing, allied health, social work, or community services).
* Experience in care coordination, case management, or similar roles within aged care or community services.
* Ability to apply wellness and reablement principles in care planning and service delivery.
* Strong people skills to support, influence, and collaborate with a diverse range of stakeholders.
* Well-developed planning and time management skills, including the ability to manage competing priorities.
* Understanding of the needs, preferences, and challenges faced by older Australians, including those with dementia, disabilities, and their carers.


Preferred Candidates

Preferred candidates will be required to undertake a National Police Check as part of the recruitment process (funded by the City).
