Job Description
We are seeking an experienced Case Manager to join our Aged Community Care Team, covering the Northern and Western Suburbs. The Case Manager will work holistically, using their relationship building skills to manage person-centred Care Plans and achieve individual goals.
The successful candidate will be responsible for undertaking case management including complex care requirements, conducting assessments to evaluate client abilities and monitor costs of Care packages, developing care plans according to client needs and ensuring compliance with policies and procedures.
Key responsibilities include:

Required Skills & Qualifications
We are looking for someone with a strong background in the Aged Care Industry, recent experience in Home Care Packages and the following skills:
* Strong communication and engagement skills
* Solid Case Management experience
* Experience creating Care Plans and managing HCP budgets
* Knowledge of service provision, co-ordination and compliance with contractual obligations
* Decision making and problem-solving skills
* Computer literacy in word processing, spreadsheets and calendar management software
