An exciting opportunity has become available for a Youth Access Team (YAT) worker to join the dedicated and vibrant team at Central Coast Local Health District.
**About You**
- You will have extensive experience working with young people (12 - 25 years), along with demonstrated knowledge of and sensitivity to the mental health needs of young people and their families.
- You will be able to maintain connections and networks with various government, community, and corporate stakeholders to establish clear referral pathways for young people in crisis and those at risk of mental health and substance abuse issues.
**Where You'll Be Working**
This role is primary based at Gosford Hospital, however you may be required to work from various outreach locations.
The YAT worker will conduct initial screening assessments, provide brief interventions, and triage, facilitating referrals to support services for young people, including those at risk or experiencing mental health issues, mental illness, and/or substance abuse problems.
The YAT worker will closely liaise with mental health professionals from CYPMH, Adult Mental Health Services, and co-located partners to determine the appropriate clinical pathway or referral for young people accessing headspace services in Gosford and Lake Haven. Additional tasks include acting as the first point of contact for young people and their families, engaging with and liaising with GPs and Allied Health Professionals, collecting client minimum data sets, and performing data entry.
