Empowering Aboriginal Communities through Culturally Safe Healthcare
The Integrated Chronic Care for Aboriginal People Program aims to increase access to culturally appropriate health services for Aboriginal and Torres Strait Islander people aged 15 years and older who are at risk of or have a chronic disease.
About the Role
This position works in conjunction with the Clinical Nurse Consultant, ICCAPP, and collaborates with internal and external partners to deliver flexible, holistic, culturally sensitive services for Aboriginal clients including liaison with relevant community groups, health professionals, and partner organisations.
About You
We're looking for a motivated professional with a strong understanding of Aboriginal and Torres Strait Islander cultures. The ideal candidate will be confident in building respectful relationships and communicating effectively with Aboriginal and Torres Strait Islander individuals, families, and communities.
* Knowledge of health issues impacting Aboriginal communities across Hunter New England Local Health District
* Ability to learn and use a computer for relevant programs including those for patient records and data reporting
Candidates should possess excellent communication and interpersonal skills with a commitment to delivering high-quality healthcare services that cater to the unique needs of Aboriginal and Torres Strait Islander peoples.
As a key member of our team you will play an integral role in improving healthcare outcomes for this population by working closely with other healthcare professionals and community representatives.
