The Community Health Cardiac Nurse is a vital member of a multidisciplinary team, providing specialised cardiac care and support. This role involves assessing and educating clients participating in cardiac rehabilitation and heart failure programs, monitoring their progress, and assisting them in managing their condition effectively. By building strong connections between general practice, community health, and external providers, this role empowers clients with the knowledge and skills needed to improve their heart health and confidently manage their recovery.

What you will accomplish:
The successful applicant will make a significant difference by supporting clients to better manage their cardiac health and improve their quality of life. Through personalised education, ongoing monitoring, and collaborative care, they will help individuals regain confidence and independence following a cardiac event.
This role provides clients with rehabilitation support to reduce chance of readmissions and enhances their long-term health.
What you will bring to the team:
* At least 3 years' experience in cardiac nursing, with strong clinical and patient assessment skills.
* Sound knowledge of cardiac and/or respiratory nursing and timely client management.
* A commitment to the social model of health and early intervention, with understanding of community health roles.
* Ability to develop, deliver, and evaluate health education programs and collaborate effectively with team members.
* Flexibility, initiative in problem-solving, and relevant qualifications including registration as a Division 1 Nurse and cardiac nursing credentials.
