About the position:
Our Post Care Services program is a service being delivered in partnerships with Relationships Australia South Australia (RASA) and Anglicare SA. The Post Care program is a comprehensive support program designed to assist young people who have been under the guardianship of the Chief Executive of the Department for Child Protection. The service aims to provide short-term case management to help young people to move successfully into independence and reach their full potential.
This role is based in Port Lincoln and assists young people across South Australia who are ageing out of the child protection system, to transition to independent living; with a focus on access to practical support and living skills. They help young people map their needs and concerns, plan their life trajectory, and engage natural helpers. They do this through case management and support coordination, including screening and assessment, goal setting, referrals, advocacy, brief counselling and facilitating client led decision making processes.
This role will work closely within multi-agency teams to collaboratively support and work with the young people accessing the service. The Support Facilitator will provide culturally responsive and trauma informed responses to the individual along with advocacy and linking them to specialist support services. Engagement with the young people takes place in a variety of settings including phone calls, group sessions, face to face meetings in an outreach setting and this may include home visits.
Key Responsibilities:
* KWY Support Facilitators provide trauma informed, collaborative case management and support coordination to achieve client objectives (including assessment goal setting and case work).
* KWY support Facilitators will provide referral options and support that promotes independent living skills around financial literacy, housing access and tenancy management, social connection, respectful relationships, cultural connections, and other life domains as identified by the client.
About you:
The successful applicant will possess skills, experience, and knowledge in working with Aboriginal and Torres Strait Islander people to address Child Protection and Domestic and Family Violence concerns. The applicant will operate within a therapeutic, trauma-informed framework. The applicant would have demonstrated knowledge of case management and working with Aboriginal families.

* Competitive salary: $100,776 - $105,340 (range dependent on experience and qualifications). Classification under the SCHADS Award Level 5.1 - 5.3

Job requirements:
* You must hold a current South Australian Driver's License or a license which permits you to drive in South Australia (and be willing to drive a service vehicle).
* You must be willing to travel to other regional SA locations when required either by car or via an airline, including flying to Adelaide for training and All Staff meetings.
* Successful candidates will be required to clear and maintain probity checks including National Criminal History Record Check (Police Clearance), Working with Vulnerable People Check and Working with Children Check (applicants need to hold a current WWCC to apply).
