Our Outreach Team is Looking for a Dedicated Support Worker
We are seeking a compassionate and proactive individual to join our team as an Outreach Support Worker. In this role, you will be supporting individuals living with neurological conditions such as MS, Parkinson's, Huntington's, MND, ABI, and Stroke.
* Key Responsibilities:
* Deliver person-centered care in a group setting, focusing on the unique needs and quality of life of each client.
* Lead and support outreach activities that promote social connection and engagement among clients.
* Assist in preparing and serving meals, ensuring a welcoming dining experience.
* Follow individual care plans to meet each client's unique needs and preferences.
* Provide mobility assistance to support safe and comfortable movement.
* Stay attuned to emotional and physical needs, ensuring care is always person-centered and responsive.

* Essential Skills and Qualifications:
* A genuine passion for supporting people with disability or the frail aged.
* Strong communication skills—both written and verbal.
* Confidence using systems and technology in your daily work.
* NDIS Worker Screening Check (or willingness to obtain).
