What You'll Be Doing
* Deliver goal-driven interventions that boost independence, mobility, and overall wellbeing.
* Conduct comprehensive assessments and co-design care plans that make a measurable difference.
* Engage in multidisciplinary case conferencing with our incredible team of nurses, allied health professionals, case managers, and support workers.
* Manage your own caseload with autonomy, backed by mentoring and clinical governance support.
* Use your clinical reasoning and creative problem-solving to help clients achieve their functional goals.
* Travel within your region — we'll sort you out KM reimbursement.

Who We're Looking For
* AHPRA registered: OT, Physio, Podiatrist, Dietitian, or Psychologist.
* Social Workers with tertiary qualifications and eligibility for AASW membership.
* Clinicians who bring both clinical expertise and heart to their practice.
* Comfortable working independently while collaborating in a multidisciplinary model of care.
* Curious, compassionate, and committed to continuous professional growth.
