Latrobe Regional Health is looking for Residential in Reach Nurse Practitioner to join our growing team on a part time (0.7EFT) ongoing basis.
Latrobe Regional Health is seeking a Residential in Reach Nurse Practitioner to provide advanced, specialist nursing care within the Residential in Reach program. Operating with a high level of autonomy and clinical leadership, this role delivers comprehensive assessment, treatment, prescribing and referral services to older people in residential aged care settings, supporting best-practice care, avoiding unnecessary hospital admissions, and improving health outcomes for vulnerable older adults.

The role of the Residential in Reach Nurse Practitioner is:
* Provide advanced, timely and evidence-based clinical assessment, treatment, prescribing and referral for aged care residents within scope of practice.
* Work autonomously and collaboratively within multidisciplinary teams, including GPs, physicians, emergency clinicians and residential aged care staff.
* Order, interpret and act on diagnostic investigations and clinical findings in alignment with legislative and professional standards.
* Determine and coordinate the most appropriate care setting based on holistic assessment, resident preferences and care directives.
* Deliver clinical leadership, supporting best practice care and continuous improvement within Residential in Reach services.
About you
Ideal candidates for this role will have:
* Eligible for endorsement as a Nurse Practitioner with AHPRA (or already endorsed).
* Mater's level Nurse Practitioner qualification or AHPRA-approved qualification.
* Significant advanced practice nursing experience, with a strong background in aged care.
* Demonstrated ability to practice autonomously while working collaboratively within multidisciplinary teams.
* Highly developed clinical assessment, communication and decision-making skills.
