Based in Coorparoo, QLD
Permanent, part-time (0.6 FTE) opportunity available
SCHADS Level 4 plus Super
Salary Packaging options (up to $15,899 tax free)

Role Overview
Support Coordinator
To support people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation through:
Strengthening their ability to coordinate and implement supports in their National Disability Insurance Scheme (NDIS) individualised funding packages
Supporting their engagement with mainstream, formal and informal services
Developing capacity and resilience in their network
Recovery Coach
Developing a recovery-enabling relationship with people including their family and carers
Supporting a person to develop and implement their individual recovery plan
Coaching a person to increase recovery skills and personal capacity, including motivation, strengths, resilience, and decision-making
Collaborating with broader systems including available services and resources
Supporting people to become active participants in the NDIS
Requirements
Minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
Experience in a human services environment, particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health.
Experience in assisting participants to test their eligibility of the National Disability Insurance Scheme (NDIS) or transition them to other appropriate supports.
Highly-developed communication, interpersonal and negotiation skills including the ability to work and communicate effectively with clients, their families, community agencies and other professionals.
Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
Excellent organisational and time management skills.
Must be eligible to apply/currently hold a Blue Card
Must hold/be willing to apply for NDIS worker screening clearance
Current Drivers License
