About the Opportunity
As a Registered Nurse, you will deliver comprehensive nursing services across diverse community and functional settings while making a meaningful difference in people's lives.
Working within a supportive multidisciplinary team, you will develop and implement holistic care plans, manage complex clinical cases including wound care, and provide leadership and supervision to Health Service Team Members to ensure consistently high-quality care delivery.
Your responsibilities will include clinical assessments, care coordination, family liaison, and facilitating smooth transitions between community and respite care settings.
You will collaborate closely with the Clinical Nurse Manager and Clinical Nurses to provide person-centred care that empowers clients to achieve their best possible quality of life.
About You
As a collaborative healthcare professional, you will thrive in team environments, building strong working relationships with diverse stakeholders, from Home Support Team Members to external service providers.
You should be a compassionate advocate who can navigate complex biopsychosocial challenges while empowering clients to make informed decisions about their care and maintain their autonomy.
You will embrace continuous improvement and innovation, contributing fresh ideas to enhance service delivery while maintaining the highest standards of clinical excellence.
Above all, you must be safety-conscious and quality-focused, with a commitment to workplace safety protocols and thorough knowledge of Aged Care Quality Standards.
Registration as a Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
Current Certificate of Registration Status
National Police check
Australian Drivers licence
CPR and Manual Handling
