Delivering Person-Centered Care in Community Settings

This is a full-time, on-site position for a Community Support Worker located in Perth. The successful candidate will be responsible for providing direct support to individuals with disabilities.

About the Role:

* Providing direct support to individuals with disabilities, including assisting with daily living activities and facilitating community access.
* Implementing support plans and monitoring clients' well-being.
* Coordinating with other healthcare professionals to ensure holistic care.
* Developing and maintaining strong relationships with clients, families, and other stakeholders.
* Participating in team meetings and contributing to the development of best practice approaches.
