Position Title: Care Navigator (Peer Support Worker)


Location: Bunbury and Busselton


Employment Type: Part Time – 3 positions: Bunbury 0.9 FTE, Busselton 0.7 & 0.6 FTE

Fixed-term contract until June 2027.

Position Overview

We are looking for a Care Navigator (Peer Support Worker) to join our team and use your lived experience to support clients following a suicidal crisis. The Care Navigator plays a central role by offering lived‑experience informed guidance, emotional support and practical assistance to help clients stabilise, stay connected and transition safely back into daily life and community services through co‑developed recovery and safety plans.


Key Components of the Role

* Build trusting lived‑experience informed relationships with participants following crisis or hospital discharge
* Provide empathetic emotional support through regular contact
* Work collaboratively with participants to create or refine personalised safety and recovery plans inclusive of goal‑setting, planning, problem‑solving and develop coping strategies
* Assist with navigating mental‑health, community and social support systems
* Accompany participants to appointments when appropriate
* Advocate for participants needs within clinical and community settings
* Maintain accurate timely documentation and progress notes
* Participate in multidisciplinary meetings and case discussions
* Collaborate closely with program coordinators, clinical teams and external agencies
* Uphold trauma‑informed recovery‑oriented and culturally safe practice principles
* Support participant transitions between services to promote continuity of care.


What You Bring

* Lived experience of suicidal crisis and a willingness to use this experience to support others.
* Experience in mental health or a related area is preferred.
* Computer skills and confidence using digital systems for documentation and communication.


Essential Requirements

* Have lived experience of suicidal crisis.
* Working towards a Certificate IV in Mental Health or willingness to complete this qualification (traineeship opportunity available).
* Experience working with minority or high‑risk populations (e.g. Youth, LGBTIQA, CaLD or people experiencing disadvantage).


Pre‑Requisites

* Current WA Driver's Licence.
* National Police Clearance.
* Working with Children Check (WA).
* Pre‑Employment Health Assessment.
