Job Title: Family Support Coordinator

This is a challenging and rewarding role that involves promoting early engagement and interagency planning with pregnant women and families at risk of their newborns entering out of home care at birth.

The coordinator will lead the overall leadership and management of the program, undertaking clinical work including coordinating, allocating and overseeing all program cases. They will also be responsible for developing and maintaining a group of appropriately skilled facilitators, undertaking facilitation roles as required, and fostering strong interagency partnerships to sustain and further develop the program.

The successful applicant will have tertiary qualifications in Social Work, providing eligibility for membership with the Australian Association of Social Workers (AASW), or current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse or Midwife, with relevant post-registration qualifications and experience working in the clinical area of their specified post-graduate qualification.

* Key Responsibilities:
* Promote early engagement and interagency planning with pregnant women and families at risk of their newborns entering out of home care at birth.
* Coordinate, allocate and oversee all PFC program cases.
* Develop and maintain a group of appropriately skilled facilitators.
* Undertake the facilitation role, as required.
* Foster strong interagency partnerships to sustain and further develop the program.
* Ongoing development, implementation and evaluation of the program.

Required Skills and Qualifications:

* Tertiary qualifications in Social Work or relevant post-registration qualifications and experience working in the clinical area of their specified post-graduate qualification.
* Demonstrated understanding and/or experience in working within an interagency context including networking and partnerships with government and non-government agencies.
* Demonstrated experience and/or knowledge in providing child centred, family focussed and person-centred care and facilitating respectful and culturally safe access to services for improving the health and wellbeing of Aboriginal families.
* Demonstrated understanding of child protection; management of child protection cases; provision of child protection consultation to clinicians/ caseworkers.
* Proven ability to lead; initiate innovative problem solving and work collaboratively with other professionals across agencies.

Benefits:

* Full-time position until 30th June 2026.
* Remuneration: $56.69 - $62.84 per hour + Superannuation.
* Evidence of current unrestricted NSW drivers licence and willingness to provide outreach throughout the Far West LHD.
