Job Opportunity:
Seeking an Advanced Nurse Practitioner to provide extended and advanced care for patients in residential aged care settings.
Role Overview:
This is a district-wide position that involves working across multiple sites, including hospitals and residential aged care facilities. The role includes hospital avoidance, hospital substitution, geriatric subacute care, and palliation.
Key Responsibilities:
* Provide evidence-based care to patients registered with the SLHD RACF Outreach Service;
* Facilitate transitions of inpatients to residential aged care prioritising those experiencing severe behavioural and psychological symptoms of dementia;
* Assist with the management of complex comorbid patients expected to transition to residential aged care in the near future.
Clinical Leadership:
* Provide clinical leadership and supervision to nursing staff within the SLHD RACF Outreach Service;
* Develop and enhance the scope of practice for Transitional Nurse Practitioners and Nurse Practitioners working within the SLHD RACF Outreach Service;
* Work in conjunction with the Nurse Unit Manager and the Medical Director to ensure all nursing staff are supported to work at their full scope of practice and access professional development opportunities.
Education:
* Develop and implement a clinical training program for Transitional Nurse Practitioners within the service;
* Work in conjunction with the Medical Director and Geriatricians within the SLHD RACF Outreach Service to facilitate supervision for Transitional Nurse Practitioners within the service.
