Clinical Care Coordinator Role Summary
We are seeking a highly skilled and experienced Registered Nurse to take on the role of Clinical Care Coordinator. The successful candidate will be responsible for leading both the clinical and care teams across the home, ensuring the needs of each resident are being met.
The Clinical Care Coordinator will play a key role in the clinical governance of the home and may be required to undertake specialist clinical portfolios to support the home's needs. Examples include education, palliative care, and infection control.
As part of an integrated team, the Clinical Care Coordinator will work within an established framework to support residents to live their best life. Key responsibilities include:
* Ensuring that clinical and any specialised nursing care needs of residents are being met and documented in individual care plans by clinical staff
* Maintaining residents and their loved ones at the centre of decision making, considering their needs, choices, independence, dignity, and identified risks involved in care planning
* Overseeing and implementing scheduled mandatory and voluntary ACFI reappraisals as required
* Holding RNs and ENs accountable for daily case reporting, care planning, and addressing patient concerns on a day-to-day basis
* Ensuring regular clinical sweeps are done and that all gaps are dealt with in a timely manner
About You
To be successful in this role, you will need:
* Current registration as a Registered Nurse with AHPRA
* A minimum of 3 years' experience in a management role within the aged care sector with a successful and proven track record
* Knowledge and understanding of the aged care sector, including the ability to identify changing trends and ensure best practice
* Understanding of and ability to manage human resource issues with effective negotiation skills
* An effective listener with the ability to communicate at all levels both orally and written
* Working knowledge of WH&S and Infection Control
