Talent Acquisition and Community Support Specialist
We are seeking a highly skilled professional to deliver direct, frontline services to individuals facing complex physical, mental health, and social challenges.
This role involves conducting comprehensive assessments, co-designing care plans, and providing assertive follow-up, including home visits, to support recovery and long-term wellbeing.
Working within a medically led multidisciplinary team, you will have the opportunity to shape clinical procedures, strengthen referral pathways across community services, and contribute to team operations and reviews.
The ideal candidate will possess tertiary qualifications in Social Work or Occupational Therapy or Rehabilitation Counsellor, and demonstrated clinical skills in assessment, planning, and evaluation of psychosocial interventions with adult patients.
You will also need experience working with patients with drug and alcohol issues, including clinical response and management of trauma, grief, and loss.
