Location South Eastern suburbs,Bayside, Mornington Peninsula, South Eastern Suburbs,Gippsland.
In this role you will work closely with NDIS participants, internal and stakeholders.
Key Responsibilities:
* Assist NDIS participants in understanding and implementing their NDIS plans.
* Coordinate and connect participants with appropriate service providers and community resources.
* Support participants in building their capacity to independently manage their supports.
* Monitor plan progress and ensure services align with participants’ goals.
* Maintain accurate records and documentation in line with NDIS requirements.
* Collaborate with participants, families, and other stakeholders to ensure a seamless service experience.
* Advocate for participants’ needs and support them in navigating challenges within the NDIS framework.
* Ensure compliance with all NDIS policies and guidelines
About You
To be successful in this role, you will have:
* Ideally an understanding and experience in community services, disability and case management.
* Excellent interpersonal and communication skills to engage effectively with participants and stakeholders.
* Strong problem-solving skills and the ability to work independently as this is a hybrid position.
* Ability to multitask and manage time efficiently in a fast-paced environment.
* Compassionate, professional, and dedicated approach to supporting people in the disability sector.
Qualifications
* Relevant qualifications in Disability, Community Services.
* NDIS Worker Screening Check (or willingness to obtain).
* Previous experience within NDIS Space (advantage).
* Valid Driver’s License and access to a reliable vehicle.
