Occupational Therapist - Home Care & Community Services

We're seeking a dedicated Occupational Therapist to provide high-quality, person-centred therapeutic services across our home care and community programs in Tasmania.

In this rewarding role, you'll work as part of a multidisciplinary team to assess client needs, implement goal-focused interventions, and support meaningful engagement in daily living. If you're passionate about empowering people to live their best life at home, we'd love to hear from you.


Key Responsibilities

* Conduct holistic OT assessments addressing physical, psychological, functional, social, and environmental needs.
* Deliver evidence-based occupational therapy interventions to enhance independence in personal, domestic, and community activities of daily living.
* Provide recommendations for home modifications, equipment, assistive technology, and adaptive strategies.
* Support clients through education, tailored therapy plans, and ongoing follow-up.
* Work collaboratively within a multidisciplinary team to maximise clients' skills and promote safe, independent living.
* Liaise with families and carers to provide guidance on effective support strategies.
* Maintain accurate documentation, client statistics, and reporting in line with organisational and professional standards.
* Contribute to continuous improvement, service development, and quality initiatives.
* Participate in supervision of students and support staff where required.
* Promote safety and wellbeing by adhering to OH&S policies and best practice.


Requirements

Essential:

* Bachelor of Applied Science (Occupational Therapy) or equivalent recognised qualification.
* Registration with relevant professional bodies.
* Demonstrated ability to assess, implement, and evaluate therapeutic interventions.
* Strong communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
* Commitment to delivering high-quality care for older people and individuals living with disability.
* Current Driver Licence.

The ideal candidate will be based in Tasmania

Desirable:

* Experience in community-based OT, home care, or aged care settings.
* Knowledge of assistive technology, functional assessments, and home modifications.
* Experience in client education, carer liaison, or health promotion.
* Ability to contribute to service planning and development.
