About the role
We are seeking an experienced Telehealth Occupational Therapist to join Constellation as an independent contractor, supporting children, NDIS participants and older adults across a diverse range of presentations.
This role is well suited to an OT who enjoys:
* conducting Functional Capacity Assessments (FCAs)
* completing functional and ageing-in-place assessments
* developing clear, practical therapy plans
* working collaboratively with Allied Health Assistants (AHAs) who implement day-to-day supports
You will focus on assessment, clinical reasoning and planning, while AHAs support carryover and implementation.
This is a telehealth-only role, offering genuine flexibility and autonomy.

The work you'll be doing
* Completing NDIS Functional Capacity Assessments (FCA)
* Conducting functional and ageing-in-place assessments for older adults
* Developing therapy plans and recommendations for AHA-led implementation
* Providing telehealth OT sessions where clinically appropriate
* Collaborating with families, carers, support coordinators and clinicians
* Writing clear, functional reports for NDIS and aged-care contexts
* Contributing to funding reviews and ongoing planning
About you
You'll be a good fit if you:
* Hold a Bachelor or Master of Occupational Therapy
* Have current AHPRA registration
* Work as an independent contractor (ABN required)
* Have experience with NDIS and/or older adults
* Are confident conducting assessments and writing reports
* Are comfortable delivering services via telehealth
* Value working independently while being part of a collaborative team
* Hold (or are willing to obtain):
* NDIS Worker Screening Check
* Working With Children Check
Experience in aged care, disability, or mixed lifespan caseloads is highly valued.
