Care Coordinator Role
As a care coordinator, you will play a pivotal role in leading and directing client care initiatives. This involves overseeing the overall client experience, fostering collaboration with our team to ensure seamless service delivery.
This position is based in our office and requires championing the best interests of our clients while contributing to business goals.
* Client goal planning, management, coordination, and review
* Active communication with team members and clients to develop care plans
* Formalising agreements with clients and their families regarding care services and costs
* Documenting interactions with clients, families, or other stakeholders and sharing relevant information with team members
* Meetings with clients and their families to discuss and review current care plans and goals
