Summary:
We are seeking a dedicated Multi Skilled Support Worker to join our team at Chorus Peel Central, supporting customers in Mandurah, Halls Head, Erskine and Dudley Park. In this role, you will provide essential support to individuals, enhancing their quality of life and promoting independence. Your responsibilities will include assisting with daily activities, offering companionship, and ensuring a safe environment. Responsibilities:
* Assist clients with daily living activities including Domestic assistance, Personal care, meal preparation, Social support, and medication management.
* Provide companionship and emotional support to clients, fostering a positive and encouraging environment.
* Ensure the safety and well-being of clients by maintaining a clean and organized living space.
* Collaborate with healthcare professionals and family members to develop and implement care plans.
* Document client progress, observations, and report any significant changes in health or behavior to the appropriate personnel in a timely manner.
* Participate in training and development programs to enhance skills and knowledge in the field of social care.
Qualifications:
* Certificate III in Individual Support or equivalent qualification in social care.
* Demonstrated experience in a support worker or similar role (minimum 1 year preferred).
* Strong communication and interpersonal skills to effectively interact with clients and their families.
* Ability to work independently and as part of a multidisciplinary team, demonstrating flexibility and adaptability in a dynamic environment.
* Compassionate and patient demeanor with a genuine desire to help others.
* Current First Aid and CPR certification (required).
