The Facility
Community nurses provide 24‑hour in‑home nursing care in the Geelong region, working closely with hospitals, allied health professionals, general practitioners and community organisations to support clients in their home environment.
The Role

An integrated nursing team supports nurses clinically and professionally with:
- An extensive structured orientation and onboarding period.
- An assigned experienced preceptor/mentor.
- Community Nurse educator.
- Ongoing education and professional development.
- Supernumerary days within the broader community health programs.
- Professional Development leave to build foundational knowledge and skills in home‑based nursing.
- Regular debrief and feedback sessions.
Responsibilities
- Conduct comprehensive client assessments.
- Acute and chronic wound assessment and management.
- Diabetes education and monitoring.
- Continence management.
- Hospital in the Home service.
- Post‑acute care service.
- Support and care of the terminally ill (in‑home palliative care).
Community Nursing Specialist Year Roles
- North – Based at the McKellar Centre – 68 hrs a fortnight.
- East – Based at Newcomb – 64 hrs a fortnight.
- South – Based at Belmont – 68 hrs a fortnight.
Roster
4‑week rotating roster including weekend shifts and afternoons:
- Day shift: 07:30 – 16:00 hrs.
- Afternoon shift: 13:00 – 21:30 hrs.
All shifts commence and finish at your assigned Community Health Centre. Vehicles, phones and personal duress alarms are provided.
Qualifications and Experience
Desirable: 3+ years of acute nursing skills and experience. The position requires a motivated, enthusiastic individual committed to ongoing professional development, with sound critical thinking skills aligned with Barwon Health values.
