Position Classification: Social Worker Level 3, Registered Nurse, Occupational Therapist Level 3, Physiotherapist Level 3, Psychologist, Speech Pathologist Level 3

Location: Negotiable across the New England North West | Kamilaroi/Ngarabal/Nganyaywana Countries

Remuneration: $74,317.93 – $120,680 per annum (dependent on qualifications) + superannuation + salary packaging

About The Service

This role sits within a dedicated multidisciplinary team providing health and development assessment, liaison and planning for children and young people in Out of Home Care. The team is passionate about improving health outcomes for vulnerable children and young people. Working across the Local Health District, the service delivers holistic, trauma-informed care and collaborates closely with carers, caseworkers and support agencies to ensure children receive the health advocacy and coordinated care they need. About The Role

As a Health Case Manager, you will provide comprehensive, trauma-informed health assessment and care planning for children and young people in Out of Home Care. Your week will involve working directly with children, carers and caseworkers, conducting assessments, developing health management plans, delivering health education and advocating to address identified health needs. The team works across the District, providing you with opportunities to travel, build strong community networks and engage with a wide range of service providers. This is a dynamic and rewarding role where your clinical expertise directly contributes to improved health outcomes for vulnerable young people.

Essential Requirements

What We're Looking For

* Degree in Social Work, Nursing, Occupational Therapy, Physiotherapy, Psychology or Speech Pathology.
* Eligibility for membership with the relevant professional association or full registration with AHPRA.
* Minimum three (3) years post‐graduate experience relevant to working with children and young people.
* Current NSW Driver's Licence and willingness to travel, including long distances as required.

Support and Development

You will be supported within a multidisciplinary team that includes a Paediatrician, Nursing staff, Allied Health clinicians, Case Managers and Administration support. Comprehensive orientation and training will be provided, along with ongoing professional development opportunities to support your clinical growth and career progression.

People (C/YP) entering statutory Out Of Home Care (OOHC) who are eligible for the OOHC Health Pathway within HNE LHD.
