We are currently looking for a Full-Time Support Coordinatorwith NDIS experience to join us in our **Dapto office.**
**The Opportunity**
As a support coordinator, you are responsible for delivering quality services and building rapport with our NDIS participants and their families
You will be working with a passionate and supportive team who are committed to working together and supporting you in your role while you undertake meaningful work that enriches the lives of people within our community.
Key Responsibilities:
- Build and maintain strong relationships with all stakeholders including government and non-government bodies, service providers, allied health professionals and practitioners.
- Coordinating and facilitating supports and services for participants in order to achieve their goals.
- Monitoring and reviewing service plans, goals and budgets in consultation with clients and representatives on a regular basis
- Supporting participants to understand their plan and supports available to them
- Coaching participants and family members on related matters
- Identifying any risks or barriers and providing solutions
- Prepare plan review reports
- Creating NDIS plans & assisting participants to manage their NDIS funds.
- Developing recovery-enabling relationships and providing advocacy as required
- Supporting the person with their recovery planning
- Coaching to increase recovery skills and personal capacity, including motivation
- Strengths, resilience and decision-making
- Collaborating with the broader system of supports to ensure supports are recovery oriented
- Supporting engagement with the NDIS, including support with plan implementation
- Documentation and reporting.
**Requirements**:
- Previous Support Coordination experience
- Qualification in Human, Community and/or Disability Services
- NDIS Worker Screening Clearance Check and WWCC or eligibility to appy
- Highly effective interpersonal skills with demonstrated experience conducting interviews/assessments with people with disability and their families/carers
- Full Australian working rights
- NSW Driver's license

Pay: $75,000.00 - $80,000.00 per year
