As a Case Manager, you will play a vital role in helping people navigate complex systems and access resources that promote their health and wellbeing. You will work collaboratively with clients, community organizations, and other stakeholders to develop and implement personalized plans that address the unique needs of each individual.
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as a strong understanding of the social determinants of health. You will also require experience working in a human services or homelessness setting, as well as knowledge of relevant models of intervention and advocacy.
In addition to your professional expertise, you will bring a compassionate and empathetic approach to your work, recognizing the dignity and worth of every person. You will be committed to creating a safe and supportive environment, free from discrimination and stigma.
What You'll Do
* Deliver direct interventions to address the complex needs of people experiencing homelessness.
* Develop and maintain relationships with community organizations and stakeholders to enhance service delivery and improve outcomes.
* Provide education and training to clients and staff on topics related to homelessness and mental health.
You'll also have opportunities to contribute to policy development, research, and program evaluation, helping to shape the direction of our organization and inform best practices in the sector.
What You'll Bring
* Relevant tertiary qualification in Human Services (Homelessness Sector) or equivalent essential.
* Experience in working in a human services or homelessness setting and knowledge of a range of appropriate models of intervention.
* Exceptional advocacy skills: You're not afraid to speak up and fight for what's right.
