Your New Role
As a Nurse Navigator – Disability, you will provide advanced clinical leadership and coordinate complex care for patients across the Logan Hospital catchment. This role focuses on improving patient outcomes, streamlining services, and creating a more integrated healthcare journey for people with disability and their families.
Responsibilities
* Provide advanced nursing leadership and expertise in disability care.
* Coordinate patient-centred care across primary, secondary, and tertiary providers.
* Engage patients and families in self-management, advocacy, and informed decision-making.
* Drive quality improvement, research, and system innovation to improve outcomes.
About you
We are seeking a highly skilled and compassionate nurse who thrives in complex environments and is committed to advocating for people with disability.
* Current AHPRA registration as a Registered Nurse with extensive clinical experience.
* Advanced knowledge of disability care, including NDIS, guardianship, and restrictive practices.
* Proven leadership, problem-solving, and change management skills.
* Strong communication and ability to build collaborative partnerships across healthcare teams.

Logan Hospital is the major health centre for one of the fastest growing areas in Queensland. We offer a range of services, including medical, rehabilitation, maternity, surgical and paediatric care.
As a teaching hospital, we help train future health care professionals and participate in research through strong partnerships with Queensland's leading medical schools, universities, and TAFE Queensland. By 2027, we'll have more beds and 5 more birthing suites as part of the Logan Hospital expansion.
