With a strong focus on independence and inclusion, they deliver Supported Independent Living, community access, NDIS coordination, and therapy supports tailored to each individual's goals and needs.
About the Role:
Join a compassionate and supportive team committed to improving the quality of life for older adults. This role offers a balanced caseload of community and residential aged care clients, allowing you to apply your clinical skills across various settings. You will help enhance mobility, independence, and wellbeing through tailored interventions and assessments.
Key Responsibilities:
* Conduct functional assessments, mobility reviews, and equipment prescription
* Provide pain management and falls prevention interventions
* Develop and implement goal-oriented care plans in collaboration with clients and families
* Maintain accurate clinical records and meet documentation standards
* Collaborate with a multidisciplinary team to ensure best-practice care
What You'll Bring:
* AHPRA registration as an Occupational Therapist
* Experience or strong interest in aged care or community health
* Valid driver's licence and own car (travel reimbursed)
* Excellent communication and interpersonal skills
* Commitment to delivering high-quality, person-centred care
